Captain James Maxwell KELLY DSC RAN
He was the NO of HMAS Melbourne when she collided with HMAS Voyager in February 1964, resulting in the loss of ‘Voyager’ and 82 lives. Any criticism which may have been implied against the ‘Melbourne’s’ command team was refuted by the second Royal Commission, and papers showed that Kelly enjoyed the Navy Board’s confidence He went on to be DNI in Canberra, Captain of HMAS Watson, and DNO in Canberra
